All employers under OSHA jurisdiction, including those exempt from routine recordkeeping, must report severe work-related events directly to OSHA. A work-related fatality must be reported within eight hours, and an in-patient hospitalization, amputation, or loss of an eye within 24 hours. Reports can be made by phone to the nearest area office, the OSHA hotline, or online. Employers that cannot reach OSHA must keep trying until a report is made.
